Inquire About Our Program
We encourage all interested parties to schedule a tour of the facility before moving forward with the enrollment process. CLC has several eligibility requirements that must be met before enrolling.
Participants must:
- Be able to voice their own needs
- Be able to feed themselves
- Be able to use restroom with little or no assistance
- Be able to follow simple directions and participate in activities
- Not exhibit constant signs of wandering
- Not use inappropriate behaviors, language, etc;
- Not exhibit combativeness or a threat towards self, other participants, or staff
Admission
Step 1 - Request an enrollment packet
Interested parties may request an enrollment packet by mail, via email, or by coming to CLC in person. (We encourage interested parties to schedule a facility tour or initial meeting before requesting an enrollment packet) Once a participant receives the enrollment packet, they will then fill-out all paperwork and forms accurately and completely and return to the center before moving forward with the admission process. The enrollment packet includes forms for both the caregiver and the physician to fill-out. Each participant must have a physical and TB-skin test administered and read before enrolling in the program. Once all necessary paperwork has been completed and returned, the participant may then schedule a pre-admission assessment with the Health Care Coordinator. - TB Test, Medication Administration, Diet Order
- Medicaid FL2 Form (must be picked up from CLC or Physician’s Office)
Step 2 - Schedule a pre-admission assessment
The pre-admission assessment requires a non-refundable $25 application fee due at the time of the assessment. If the participant is deemed appropriate for the program, the Health Care Coordinator will then move forward with the admission assessment.
Step 3 - Schedule a start-date
After all assessments have been completed and a level of care has been determined, the participant and caregiver will then meet with the Executive Director to setup a tentative schedule/start-date and to discuss payment options.
