Funding

Creative Living Center offers a low-cost alternative to nursing or rest home placement. Through funding and contributions from agencies such as United Way, Mideast Commission, Department of Social Services, and more, CLC is able to offer many opportunities for participants to receive free or reduced cost services. CLC offers services to adults on both grant and private pay basis.

Grant opportunities:

  • HCCBG (Home and Community Care Block Grant) - This grant is funded through the local Mid-East Area Agency on Aging and is available to participants 60 and older who can exhibit a need for financial assistance. CLC is allocated a set number of units and is responsible for awarding these grant spots based on both need and waiting list priority.
  • SSBG (Social Services Block Grant) - is awarded to CLC participants by Pitt County Department of Social Services. For more information on the requirements for this grant, please contact DSS at 902-1111.
  • CAP/DA (Community Alternatives Program for Disabled Adults) - funded by Medicaid, this grant is only eligible to persons possessing the Medicaid “blue card.” Call 902-1111 for more information.

Private Pay:

Because grant opportunities have restrictions and are often wait-listed, CLC offers private pay plans for eligible participants. Private pay fees range from $22 - $46 per day and are based on the participant’s level of care and individual income. This daily rate includes all needed supplies and meals. Half day rates are also available upon request. Scheduled lunch and community outings are additional.
